How to use these tools

These assessments are starting points — not diagnoses. We encourage you to take the assessment in a quiet, honest moment with yourself. Review your results without judgment — curiosity, not criticism. Bring your results to your next therapy or coaching session to explore further. PHQ-9 — Patient Health Questionnaire (Depression)

A widely used, validated 9-item screening tool for depression severity. Takes approximately 2 minutes to complete.

GAD-7 — Generalized Anxiety Disorder Scale

A 7-item validated screening tool measuring anxiety severity across the past two weeks.

PCL-5 — PTSD Checklist for DSM-5

A 20-item self-report measure assessing PTSD symptoms as defined by the DSM-5. Developed by the National Center for PTSD.

PHQ-4 — Ultra-Brief Depression & Anxiety Screen

A 4-item combined screener for both depression and anxiety — ideal as a quick intake tool.

Columbia Suicide Severity Rating Scale (C-SSRS) — Screener Version

A brief, validated tool for assessing suicidal ideation and behavior. For clinical and self-awareness use.
