Our licensed clinicians conduct thorough, confidential DWI and DUI screenings and assessments that meet New York State requirements and are completed with the professionalism and compassion our clients deserve.   • A screening is typically the first step in the evaluation process and is often required following a first-time offense or as part of a diversion program. It involves a structured clinical interview and standardized assessment tools to determine the nature and extent of any alcohol or substance use and its relationship to the incident in question.

    Screenings at Awaken Wellness include:

    • Clinical interview reviewing substance use history, frequency, and patterns

    • Use of validated screening instruments such as the AUDIT, DAST, or CAGE

    • Review of relevant legal and background information

    • A written summary report suitable for court, DMV, or program submission

    • Referral recommendations where clinically indicated

  • For individuals requiring a more in-depth evaluation, including those with prior offenses, complex substance use histories, or court-mandated comprehensive evaluations, our full assessment provides a detailed clinical picture that goes beyond the screening level.

    The comprehensive assessment includes:

    • Extended clinical interview covering substance use, mental health, trauma history, and psychosocial functioning

    • Multiple validated assessment instruments

    • Review of legal documentation and prior treatment history where applicable

    • Diagnostic impressions in accordance with DSM-5 criteria

    • A detailed written report with findings, clinical recommendations, and level of care determinations

    • Collaboration with legal counsel, probation officers, or treatment providers as authorized

  • Individuals seeking to restore their driving privileges following a license revocation or suspension may be required to complete a substance use evaluation as part of the DMV relicensing process. Our clinicians are experienced in completing evaluations that meet New York State DMV standards and can provide the documentation needed to support your application for relicensing.

Before Your Appointment

Once your appointment is scheduled, we will provide you with a list of any documents to bring — such as your legal paperwork, court order, or DMV correspondence. Having these on hand helps ensure your evaluation is completed accurately and that your report reflects all relevant information.

Your Evaluation Appointment

Appointments are conducted in a private, confidential setting by a licensed clinician. You will be asked questions about your substance use history, the circumstances surrounding your charge, your mental health history, and your current life situation. There are no trick questions, we are gathering information to form an accurate clinical picture, not to build a case against you.
Depending on the type of evaluation required, your appointment may last between 60 and 120 minutes.

Your Report

Following your evaluation, a written clinical report will be prepared and provided to you and, with your written authorization, to the appropriate court, DMV office, attorney, or program. Reports are completed in a timely manner — we understand that legal timelines are often pressing and we treat your deadlines as our own.

Recommendations & Next Steps

If the evaluation identifies a clinical need for treatment, education, or additional support, your clinician will discuss those recommendations with you directly and provide referrals as appropriate.

Screening & Assessment — $125

A 90-minute intake session designed to understand your needs, history, and goals, and to determine the best path forward together. This thoughtful first step ensures the work is paced and tailored to who you are.

A few things to know:

  • A free 15-minute phone consultation is available prior to scheduling

  • Payment of $125 is due at the time of service

  • DWI and DUI evaluations are not covered by insurance
    — however we will provide a receipt for your records

  • We will communicate directly with your attorney, probation officer, or court as needed with your written authorization
