Do you need a letter for a DWI/DUI case?
Chanel Tazza is certified by the New York State Office of Alcoholism and Substance Abuse Services to provide clinical screenings and assessments for people arrested for impaired driving and, if necessary, the provision of treatment.
The Process & How Payment Works
Service Requirements:
We will need Identification to document NYS License #
A copy of the arrest report (obtained from the police station of the arresting officer)
Any documentation provided to you by the courts
Cost of Services:
The fee for a screening or assessment visit is $100.00. Depending on your insurance type this may be a covered service.
